Candace Chitty is President and CEO of Quality First Healthcare Consulting, Inc. (QFHC) and Chief Operating Officer for Quality First Management Solutions, Inc. (QFMS). Through her leadership consultants provide operations management consulting services across the US delivering effective results to organizations seeking to improve the quality of health care they deliver by making improvements in coordinating the utilization of services, population health management, document management, outcomes measurement and reporting.
Candi brings over 26 years experience to our healthcare industry. Ms. Chitty holds a Masters Degree in Business Administration in Health Care Management, a Bachelors’ degree in Nursing, a National Certification in Healthcare Quality, and is licensed in the State of Florida Healthcare Risk Management.
She is an active member of the Case Management Society of America, (CMSA), American Evaluation Association (AEA) the National Association of Community Health Centers (NACHC), and the Patient-Centered Primary Care Collaborative (PCPCC). She is an independent NCQA surveyor for NCQA’s Health Plan Accreditation, Patient-Centered Medical Home, and Credentialing Vendor Organization products.
Starting her consulting business in 2001 with a focus on NCQA accreditation project management, she has expanded her management consulting to include health plans, physician groups, hospital systems, community health centers, state and federal funded health care programs, pilots and demonstration projects, and HIT companies. In 2009 she and her business partner formed Quality First Management Solutions, Inc, a sister company to QFHC and the information technology business component.
She is accomplished in the NCQA survey review process having been involved since 1998 in NCQA survey preparation management and more recently being invited to conduct survey reviews as an external surveyor for NCQA.
Other key accomplishments include:
- Developed and successfully implemented Emergency Department Diversion Programs in three states by building collaborating partnerships between hospital systems and community health centers.
- Lead designer of rule-based care management software for the leading provider of software solutions for collaborative care management.
- Developed the on-line advanced technical certificate (ATC) in Case Management for the largest community college in the United States.
- Developed a risk based integrated population health program model deployed to Physician Hospital Organizations (PHOs) and self-insured employer groups.
- Lead designer of PolicyPlus™, a web-based document management and credentialing software application.
